By completing this form ahead of time, you will:
1. Provide us with a snapshot of your marital estate for the attorney-mediator to review ahead of time.
2. Participate in the process of financial disclosure, an important part of a successful mediation.
3. Ensure that you leave your consultation with a defined course of action.
The form takes about 10-20 minutes to complete.

However, you can save and resume the survey later. Upon clicking the "save and resume later" link, you will be prompted to enter a password of your choice and then provided a personalized link to retrieve your survey within the next 30 days. Please save this link and password as they are required to resume the survey. The survey takes about 10-20 minutes to complete.
Prior to completing the survey, we recommend having all of the account names and estimated values* for the following property types in front of you:

Estimate Values are Used for the Consultation Only.
Estimated values are used as a preliminary step toward voluntary disclosure and to help the attorney-mediator prepare for the consultation only. The estimated values do not need to be 100% accurate, nor are they expected to be. The most important part of this step is to fully list all the property, accounts, debts, etc that will need to be discussed and negotiated. If there is any concern about the estimated values being grossly inaccurate or misinterpreted, you may put $1.00 as a placeholder. That being said, note that the mediation process does require that actual values of each account be fully disclosed. Also, while password and full account numbers are not required, account statements must be made available. At the same time (and equally important), our mediation process requires and includes legal education for spouses on how marital property in PA is valued and divided so they can make informed realistic proposals of one another. All of this is discussed at length in the consultation.
